Empire to require Tax statements each year

 

Empire is implementing changes to its small group re-credentialing process.   Effective immediately, as part of Empire’s re-credentialing process, small groups are required to submit wage and tax documentation and other necessary documentation to verify small group size and group eligibility. The Small Group Re-credentialing form will no longer be used by Empire.  Brokers and groups should no longer submit this form to Empire.   Re-credentialing documentation will be requested from small groups by letter 60 to 90 days prior to the group’s renewal date.
